Another important part of safe aging is evaluating your safety. Our Safe At Home program is specifically designed for individuals in need of care management. The Safe At Home program offers in-home care recommendations for you or your loved one for safe daily mobility and care.
Safe At Home Includes:
- Home safety evaluation
- Full health assessment by a Registered Nurse
- Service Care Plan preparation
